"NEW YORK STATE GUIDELINES"

REGARDING WHETHER

TO SEND YOUR CHILD TO SCHOOL OR NOT

 

 

 

PARENTS,

PLEASE GIVE US A COURTESY CALL IF YOUR CHILD IS GOING TO BE ABSENT FROM SCHOOL DUE TO AN ILLNESS.

PLEASE NOTE, IF YOUR CHILD IS RUNNING A TEMPERATURE OF 100 DEGREES OR MORE, HAS A PERSISTANT COUGH, VOMITS, HAS LOOSE BOWELS (DIARRHEA), PINK EYE, OR AN UNKNOWN RASH WE WILL NEED TO CALL YOU TO PICK UP YOUR CHILD.  WE DO UNDERSTAND THE INCONVENIENCE THIS MAY CAUSE, HOWEVER, WE MUST ADHERE TO THE "NEW YORK STATE GUIDELINES". YOU WILL RECEIVE THIS FORM DURING OUR "OPEN HOUSE".  IF YOU ARE UNABLE TO ATTEND, THE FORM WILL BE HANDED TO YOU THE FIRST DAY YOUR CHILD COMES TO SCHOOL.

YOUR CHILD MUST BE SYMPTOM FREE FOR 24 HOURS BEFORE RETURNING TO SCHOOL.  A DOCTOR'S NOTE (ON THEIR LETTERHEAD) IS REQUIRED FOR PERSISTANT COUGHS, PINK EYE AND ANY RASHES UPON RETURNING TO SCHOOL.  (FAXED NOTES) WILL NOT BE ACCEPTED.

IF YOUR CHILD NEEDS MEDICATION (PRESCRIBED OR OVER-THE-COUNTER) GIVEN AT SCHOOL, YOU MUST HAVE YOUR CHILD'S PEDIATRICAN FILL OUT  OUR PARENT/PRESCRIBER MEDICAL FORM.  SHOULD YOUR CHILD NEED TO TAKE MEDICATON DAILY, THE PARENT/PRESCRIBER MEDICAL FORM EXPIRES SIX MONTHS FROM THE DAY THE PRESCRIBER SIGNS THE FORM AND, THEREFORE, MUST BE RENEWED EVERY SIX MONTHS.  FORMS ARE AVAILABLE UPON REQUEST.
